Position Title: HIV Care and Treatment Nurse
Reports to: HIV Coordinator

Position Overview:
The HIV care and treatment nurse will lead the linkages systems in HIV implementation at the PHC level in collaboration with MOH staff. Under the supervision of HIV Coordinator, the HIV care and treatment nurse is responsible for providing direct service delivery in MNCH, adult and pediatric ART services. Additionally, role will be to participate and strengthen Quality improvement initiatives at the PHC level. Works in collaboration with the TB/HIV Lead to ensure delivery of quality HIV care and treatment at Pleebo Health Centre.
Essential Duties and Responsibilities:
  • Provide direct service delivery in the ART clinic according to a structured schedule focusing 95-95-95 indicator performance.
  • In collaboration with the HIV Coordinator, the nurse will ensure eligible clients access Viral Load Testing and those with detectable VL are provided with quality Intensive adherence counselling and followed up to finish the whole cascade of care.
  • With the support of the NACP mentor the nurse will strengthen, scale-up and track the performance of mother-infant pair (MIP) as one of PIH differentiated service delivery model.
  • Support and strengthen integration of PMTCT and EID in MNCH.
  • Work with MOH staff to strengthen the provision of Family Planning services in ART clinic.
  • Support MOH and PIH community team to implement community programs on the linkage of all HIV clients to psychosocial support groups.
  • Promote retention strategies through implementation of the appointment registers and early tracking of missed appointments in collaboration with CHV’s and CHP’s.
  • Liaise with laboratory to ensure viral load coverage, facilitate screening of clients to identify eligible clients for VL testing, and follow ups of detectable VL in collaboration with Community team.
  • Assess NCDs (Diabetes, Hypertension, Depression and Substance Abuse) among PLHIV and link to NCD and mental health clinics.
  • Support CQI initiatives by playing a leading role in follow up of action points and collaborating with the TB/HIV Lead on rapid learning cycles to ensure achievement of the targets set.
  • Support the management of Advanced HIV Disease (AHD) by strengthening screening of eligible recipients of care, facilitating lab tests and documentation of results.
  • Coaching of the CHV’s and CHP’s, mentor mothers on the linkage systems and correct documentation.
  • Participate in the data collection, documentation and sharing of promising practices related to PMTCT, EID, adult and pediatric HIV interventions.
  • Participate in the writing of dHTS and ART reports on monthly, quarterly intervals and other periodic reports as required by PIH.
  • Track the performance of EID indicators at 6 weeks, 12 and 24 months.
  • Work with the HIV Coordinator to conduct and represent PIH in HIV care and treatment meetings, TWG with MOH staff, QI meetings, and review of indicators within the 95-95-95 framework.
  • Support Maryland NACP mentor and dHTS activities and contribute on strategies on attaining CHT goals and targets.
  • Ensures self-professional development.
  • Ready to perform other duties not under the PHC as needed and assigned by PIH-L.

 

Qualifications:
  • Registered nurse, Bachelor’s in Nursing added advantage
  • More than 5 years of experience in HIV management. He/she should be trained in current ART management and registered with the relevant Registration bodies.
  • Familiarity with donor HIV program implementation, documentation and reporting.
  • Good communication skills, integrity and humility.
  • Good understanding on equity, diversity and inclusion.
  • Glebo, French languages are added advantage.
