Location: Canterbury CT1 3NG, Ashford TW15 3AA, Dover CT17 0HD.
Contract type: Permanent x 3 positions
Hours: 35 per week
Salary: £28,821 per annum based on 35 hours per week

Are you an experienced Outreach Worker within the Canterbury, Ashford or Dover areas who would love to join us to work on an innovative NHS programme that helps support people who access health care more than expected (High Intensity Use (HIU) programme)?
What does being an Outreach Caseworker involve?

You’ll work in the community to help identify and address unmet social needs that may be exacerbating physical and/or mental health.
You’ll help people with complex presentations including mental health, substance abuse, multiple long-term conditions and adverse traumatic experiences. They’re often suspicious of formality and offers of support, and your compassionate, authentic nature will help them access the help they need.
You’ll help to lower stigma within the community and change mindsets and behaviours. You’ll achieve this by building strong relationships with partners and stakeholders, such as A&E departments, Community Mental Health Teams, GP surgeries, housing teams etc.

What will you bring to the role of Outreach Caseworker?

Do you have experience in person centred support planning? Can you identify risks and anticipate issues?
Do you understand how the system works and how to help people falling through the gaps?
Do you have an exceptionally strong ability to connect with other people? Are you authentic, relatable and compassionate?
Can you think on your feet and have strong negotiation skills?
Are you resilient, motivated and courageous and have a real appetite for helping some of the most vulnerable people in our society?
Do you have meticulous record keeping skills?

Due to the specific requirements of this role, you will need to visit various locations, some of which are remote with no access to public transport. Therefore you must have a UK driving licence and access to a vehicle or be eligible for transportation via the Access to Work Scheme.
