Location: Shrewsbury
Contract type: Fixed Term to end March 2024
Hours: 35 per week (Monday to Friday)
Salary: £22,315 per annum based on 35 hours per week

As a Service Coordinator, you will oversee a team who support people who may suffer from isolation and loneliness. The focus is on tailoring support to ensure they are discharged from hospital in a timely manner and then helping them regain or maintain their independence and improve their wellbeing at home.
A day in the life of a Service Coordinator:
You will respond efficiently to referrals from Telford and Royal Shrewsbury Hospitals.
You will manage a small team of staff and volunteers ensuring discharge of patients from hospital is carried out professionally & efficiently.
You will follow up with patients after their discharge to ensure they are aware of services available to them. You will guide or signpost them to connect with groups within their neighbourhood or help them to manage a health condition. The aim is to enable them to lead independent lives whilst improving their wellbeing.
You will produce and maintain monthly reports.
To be a successful Service Coordinator:
Do you have experience in leading a team and and are capable of getting the best out of people?
Do you have a good knowledge of Health and Social Care services to ensure the best service is provided?
Do you have good communication skills?
Do you have a UK manual driving licence?
