Service Coordinator
Location: Based at St Thomas Hospital, London but travel will be required across South East London
Hours: 35 per week (includes evening shifts)
Salary: £22,781 based on 35 hours per week, plus Inner London Weighting of £3,344 if applicable
Contract Type: Fixed Term until end of March 2024 with possibility of extension
Do you have great social skills and a passion to make a difference to the lives of vulnerable people within the community?
As part of the South East London Homeless Discharge team, you will oversee staff and volunteers who support people who are homeless or at risk of homelessness.
The focus is on tailoring support to ensure people who need us are discharged from hospital in a timely manner. You will then help them regain or maintain their independence and improve their wellbeing.
A day in the life of Service Coordinator:
You will respond to referrals from St Thomas Hospital.
You will assess the needs of the people we help, completing and following a care plan for them.
You will ensure homeless patients have sufficient assistance and provisions. You will signpost them to legal and voluntary services in the community after hospital discharge.
You will escort patients to appointments and liaise with other agencies to achieve the best results for them.
You will support patients who have been discharged into temporary hotel beds in the South East London area.
To be successful in the role of Service Coordinator:
Do you have experience working with vulnerable adults? Service confidentiality and safeguarding vulnerable people are key in this role.
Do you have excellent communication and time management skills? Can you delegate and manage set tasks to agreed timescales?
Are you experienced in preparing and keeping written records and reports?
Are you confident working with NHS professionals, community organisations and care agencies?
Are you IT literate, particularly with Microsoft Office packages?
It would be advantageous if you were in possession of a Level 3 qualification in care or working towards one.

Although travel is required across South East London, no driving licence is needed.
