Service Support Worker – Assisted Discharge
Contract type: Fixed Term until 31st December 2023, with a possibility of an extension
Hours per week: 21
Salary: £11,658 per annum
Location: Daisy Hill Hospital, 5 Hospital Road, Newry, Down, BT35 8DR, with ability to travel, full UK Driving Licence required.
Since November 2019, the British Red Cross has reduce potential delayed or premature discharges and avoid unnecessary admissions and A&E attendances.
We’ve got an opportunity…
We are looking for a passionate Service Supporter Worker to join our Health and Local Crisis Response team. The Service Support Worker post will provide reactive, focused and flexible support to inpatients following a hospital stay at Daisy Hill Hospital to enable early discharge when clinical needs have been met. The Health & Local Crisis Response service supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care, and emergency services.
A day in the life of a Service Support Worker – Assisted Discharge will involve;

Assisting the Health and Local Crisis Response Service Coordinator by undertaking duties required to meet the service users’ needs in order to re-enable service users to remain in their discharge location and prevent hospital re-admission.
Maybe taking the patient home from hospital to enable their discharge.
Assessing service users’ needs, complete and follow a support plan for the individual.
Providing practical and emotional support in the service user’s discharge location and signposting to other services where appropriate to enable the individual to live independently in their accommodation
Acting as contingency for the Service Coordinator when required and may need to provide cover for their colleagues at other hospitals served by the Red Cross in the area.
Service confidentiality and safeguarding vulnerable people
Working shifts including weekend and some evening shifts (11am – 7pm)

This post does not include personal care and management of medication.
To be a successful Service Support Worker – Assisted Discharge , you’ll need;

Experience of working with vulnerable adults.
To be a good communicator who can delegate and manage set tasks to agreed timescales.
Have experience preparing and keeping written records and reports.
The ability to work with NHS professionals, community organisations, care agencies and relatives in order to support people in the community to life safely and supported at home.
A Level 3 qualification in care or currently be working towards this will be an advantage.
A Full UK driving licence.

The closing date for the role is 23.59 hours on 22nd June 2023 with interviews expected shortly afterwards.
