Support Worker (Support at Home)
Location: Shrewsbury SY1 3AF
Hours: 35 hours per week
Contract Type: Fixed Term to end March 2024 with possibility of extension
Salary: £19,838 per annum based on 35 hours per week
Do you have a genuine desire to make a difference to peoples’ lives within the community by assisting them at home?
Do you want to be part of our team who help people for up to six weeks following their discharge from hospital? You would help service users to regain their independence at home and reduce the risk of needing another hospital visit. The people you will assist will come from all kinds of backgrounds. This assistance will have a huge impact on their day.
As a Support Worker, you might be asked to collect prescriptions, make sure there is food in the house or do light household tasks. You will also make sure our service users know who to contact should they need more help – the general assistance you would hope your own family and friends would experience.
You will be a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. You’ll allow people to enjoy healthier, more fulfilling lives and is an amazing way to give back to your community.
A day in the life of a Support Worker:
You will provide practical and emotional help (telephone calls, welfare checks, appointments, shopping etc).
You will assess service users future needs and develop support planning.
You will guide and signpost to relevant agencies and community groups for assistance/engagement (financial advice, befriending services etc.).
You will ensure service users’ support plans are updated and maintained.
You will drive to different locations throughout Shropshire to fulfil service users’ needs, as required.
You will have access to a Red Cross vehicle most of the time but may occasionally need to use your own vehicle. Mileage will be paid for use of your own vehicle.
To be a successful Service Support Worker:
Do you have a good knowledge of services provided by the NHS and Social Care?
Are you caring and empathetic and able to offer the best support be it on phone or face to face?
Do you have a passion to make things better for people who rely on us?
Do you have a basic IT knowledge?
Do you have a full UK manual driving licence held for minimum of two years and access to a vehicle?
Please note there is no personal care or administering of medication in this role.
