Job Title: Support Worker
Location: Poole Hospital
Hours: 20 hours per week, working Monday to Friday 4pm to 8pm
Contract Type: Fixed Term to end March 2024
Salary: £19,838 per annum based on 35 hours per week
Driving: Full UK Manual Driving Licence to drive British Red Cross Vehicle

If you can make someone feel comfortable and at ease then you’d be perfect as a Support Worker in our Health and Local Crisis Response team.

You will provide assistance to patients to enable them to be discharged in a safe and timely manner from Poole Hospital.

A day in the life of a Service Support Worker, you will:

Act on referrals from Poole Hospital to collect patients who are ready for discharge.
Provide transport to ensure patients can be discharged and taken home.
Ensure patients are settled at home following hospital discharge. Collect prescriptions, do light household tasks or shopping if required.
Provide a support plan. Signpost patients to relevant services following a hospital stay to help them regain their independence at home and reduce the risk of hospital re-admission.

To be a successful Service Support Worker:

Do you have a full UK manual driving licence held for a minimum of two years?
Do you have a knowledge of services provided by the NHS and Social Care?
Are you thoughtful, caring and patient?
Do you have a basic IT knowledge? Training will be given on British Red Cross systems.

There is no personal care or administration of medication for this role.

DBS check will be paid for by British Red Cross and workwear will be supplied for your role.
